Exceptional health care for South Phoenix and Laveen families is now more convenient than ever with Valleywise Health’s new state-of-the-art community health center at 35th Avenue and Southern Avenue. The Valleywise Community Health Center – South Phoenix/Laveen will open its doors to patients Monday, August 17.
The new facility is conveniently located in the heart of Laveen and offers an extensive line of services, including primary and pediatric care, OB/GYN, radiology and behavioral health. In-person and telehealth appointments are available with all COVID-19 guidelines being followed for in-person visits.
“A great deal of thought went into making this right for the community it serves,” said Steve Purves, President and CEO of Valleywise Health. “Families in Laveen and South Phoenix will have access to Valleywise Health’s outstanding adult and pediatric caregivers without having to stray far from home. Our staff and District Medical Group physicians are excited to provide the care families of Laveen and South Phoenix need and deserve.”
The new clinic is part of Valleywise Health’s network of community health centers spread across Maricopa County. It is made possible thanks to voters who overwhelmingly supported Proposition 480 in 2014. The ballot measure provided funding for the transformation of the Valley’s only public teaching hospital and safety net system of care. The Laveen location and new locations in North Phoenix and Peoria will come online in November and January respectively.

The center also will host medical residents in partnership with the Creighton University Arizona Health Education Alliance.
