Equality Health, a whole-health delivery system, has been chosen as the recipient of Health Current’s 2020 Health IT Innovation Award for its leading-edge health information technology (IT) that improves care coordination and paves the way for value-based care transformation. This annual award presented by Health Current, the health information exchange (HIE) of Arizona, recognizes an organization that demonstrates innovative uses of health IT to achieve the Triple Aim of better care, better outcomes and lower costs.
“Equality Health has made a lasting impact in healthcare with their technology, which supports providers and payers in addressing patients’ needs and coordinating care for millions,” said Melissa A. Kotrys, MPH, CEO of Health Current. “They are truly innovators when it comes to advancing healthcare through health IT and HIE, and we are pleased to recognize them with this award.”
Through its proprietary technology platform, CareEmpower, Equality Health empowers payers and providers to improve quality, align with value-based principles, and understand how population-specific factors such as culture and language influence patient care and outcomes. The solution aggregates fragmented data into streamlined practice and care team workflows to close gaps in care.
“We are honored that Health Current has chosen Equality Health for this prestigious award,” said Hugh Lytle, Founder and CEO of Equality Health. “We are proud of what we’ve been able to accomplish with our people and technology and the meaningful change we’ve catalyzed in the value-based care environment.”
To date, CareEmpower has been deployed by Equality Health’s partners to support more than 14 million covered lives in Arizona and across the country. The following metrics demonstrate the platform’s impact:
- 19% improved performance in HEDIS adolescent well visits
- 10% improved performance in diabetes care testing
- 25% reduction in 30-day readmissions
- 85% decrease in post discharge follow-up time
- 90% cultural care training satisfaction
“Ultimately, payers and providers need more actionable data, greater collaboration, better patient engagement and stronger care coordination to succeed on the new frontier of value-based care,” said Doug Duskin, President of Equality Health’s technology division, “and we’ve made that possible with CareEmpower.”
