Oak Street Health, a network of value-based primary care centers for adults on Medicare, is now welcoming patients at the new Surprise center at 13030 W Thunderbird Rd. Oak Street Health is now part of CVS Health, and together, the pair are advancing primary care with a value-based approach that leads with preventative care, removes barriers in care and improves patient outcomes, particularly for communities with the greatest need. This will be the first Oak Street Health center in El Mirage and the 14th across the state of Arizona, following other locations such as Tucson, Phoenix, Mesa, Apache Junction, Chandler, Tempe, Sun City, and Glendale.

In addition to our twelve exam rooms, each center has a spacious community room where older adults can gather for social connection and participate in various activities and events. Some events that take place in the community room include bingo, arts & crafts, chair yoga and educational courses as well as other amenities including computer and internet access. Oak Street Health accepts all forms of Medicare, including Original Medicare Part B, select Medicare Advantage plans, Medicare Supplement or Medigap plans and Medicare-Medicaid Plans.
