Our Nurse Practitioners are supported by the Medical Director, Nurse Practitioner Managers, Registered Nurses, Community Health Workers as well as a back office support team.. 

Position Essential Functions

• Delivers high-quality, cost-effective care at scheduled and occasionally urgent visits
• Completes comprehensive histories and physical exams, develops assessments/diagnoses and creates, executes and evaluates treatment plans
• Focuses on social determinants of care and preventive measures (such as HEIDS) not only working with patients but also getting to know and working with families and care givers for the best possible outcomes
• Communicates with the entire PHC support team as well as insurance Case Managers and Customer Services
• Participates in grand rounds, panel rounds and multi-disciplinary team meetings as well as CME to enhance personal goals

Position Requirements

• Holds active unencumbered license in Arizona
• Board certified with AANP or AANC
• Has an active DEA license
• Current CPR certification
• Two or more years of practice experience preferred
• Must have valid state driver license and automobile insurance
