Position Summary: 

 

Ensures team provides 24/7 coverage with direct involvement when necessary.  Provides efficient clinical and administrative supervision of an Assertive Community Treatment Team (ACT).  Serves as a liaison with administration, clinical services, service providers and external stakeholders.  Reviews appropriateness for outpatient and inpatient treatment services.  Oversees team's day-to-day functioning in relation to the delivery of Assertive Community Treatment Services.

 

 

 

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES

Participates in the hiring, training, coaching, mentoring, and supervising of ACT Case Managers, and all ACT Team Specialists both administratively and clinically.  Monitors and evaluates direct reports overall performance. 

 

Responsible for ensuring team is compliant with all established fidelity measures.

 

Monitors population on civil or criminal Court Ordered Treatment (COT) to ensure that behavioral health participants and teams are adherent to all terms of the court order.  Prepares monthly COT report.  Ensures that staff communicate with courts as required either in person or written report and that such communication is consistent with the assessments, Individual Service Plans, and team recommendations for the service participant.

 

Works with the clinical team to ensure individuals receive appropriate care within the least restrictive setting including coaching and mentoring specific to inpatient and residential treatment discharge plans.

 

Ensures that direct reports are providing coordination of care to internal and external provider agencies, stat entities or other stakeholders (e.g. probation officers, advocates, family member, hospital staff, jail staff vocational rehabilitation etc.) as needed.

 

Ensures that all ACT staff maintains accurate, thorough, and current documentation in accordance with policies, procedures, and clinical protocols.

 

Ensures daily communication occurs at a level necessary to provide care for all service participants served at a level required for continuity of care across team principles.

 

Provides clinical oversight of services provided, assessments conducted, and individual service plans developed for all service participants assigned to their team.

 

Available on-call 24 hours/day, 7 days/week unless otherwise scheduled as part of a rotation or in event of a scheduled absence in which coverage has been arranged.

 

 

 

 

MINIMUM QUALIFICATIONS

 

This is a supervisory position requiring more education and experience therefore the applicant must have one of the following combinations of education, license and/or behavioral health work experience:

  • Must be a Behavioral Health Technician at a minimum with:

a)       Bachelor’s degree in a behavioral or healthcare related field and a minimum of three (3) years behavioral or healthcare work experience. OR

b)      Master’s degree in non-related field with two (2) years behavioral or healthcare work experience.  OR

c)       Bachelor’s degree in non-related field with (4) years behavioral or healthcare work experience. 

  • A Clinical Coordinator must also have one year of supervisory or management experience.

 

 

 

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Computer Skills: Proficiency in typing, using computer software, i.e., , Word, Excel, and PowerPoint and Internet is essential. Must be proficient in word processing, Word, Excel, and Power Point as well as possess the ability to learn new software systems.

 

Other:  Interpersonal skills which allow for professional and positive relationships with co-workers, consumers, families, and other community members.  Good communications skills, evidence by a non-judgmental approach, effective listening and other positive interactions
